Transferring care to another clinic
What is this form for?
If you are not returning to Skin Check WA for continuing care, and would like us to transfer your clinical information and continuing care to another medical clinic.
How this form works?
Once submitted, this will be forwarded to the treating doctor at Skin Check WA, who will organise for the file to be compiled.
At the same time, the clinic nominated will be asked to accept the request.
The file will be emailed as an encrypted pdf, after verification of the provider details.
When will the result arrive?
We aim to have the file sent within 5 business days.
Can I just call the clinic rather than go through the form?
Our receptionists and nurses are not authorised to release results or clinical notes.
Is there another faster way?
Yes, if only pathology reports are required, a registered medical practitioner can obtain the reports from the pathology provider. Our pathology provider currently is Clinipath.
